help with captcha placement

Hello -

I am a complete PHP newbie, and I so I need help!   I was able to create the image and the number generation on my contact site:
http://www.justinflynn.com/contact/contact.php

However, I cannot get the captcha placed correctly in the php file.  The emails go through unchallenged.

Perhaps someone could help? I'd like to keep the (*) fields are required, but could lose that if necessary.
***Currently the check script is not placed in the file.  Nothing I tried worked.

I am having the same problems (e.g. mail message pass no matter what is entered).  I am using formmail.pl. 

Any assistance would be greatly appreciated.
