This usually means some HTML is sent before line 67.
Trace back from line 67 and start looking for echo statements or an escape from php to html.
It can sometimes be hard to find - like a blank line at the end of a previously included file.

What do you have in line 8 in password_protect.php?

Try removing any characters (spaces, tabs, new line characters, etc) from password_protect.php, after the ending ?>
